What is supported, and what is missing
The two fictional source documents are listed separately from the patient's own description. Original imaging and the latest medicine list are still missing. The dates given in the summary and the patient's account differ; that conflict stays visible until clarified.

First obtain the missing original documents and clarify the review question. A responsible clinician must determine which specialty review is appropriate and what can be concluded from the available records. This example does not select a treatment or establish travel fitness.
